While we pride ourselves on the safety of the program, it must be recognized that any youth activity such as playing on a playground, youth sports, or any other activity involving height or motion can lead to injury. This risk can be minimized but can never be completely eliminated. Even with the best possible equipment and instruction, the possibility of minor to serious injury remians. We, the staff of Flipside Gymnastics, recognize our obligation to make our students and their parents aware of the risks and hazards associated with any physical activity or youth sport. As a parent or guardian, you also have a responsibility to make your child aware of the possibility of injury and encourage your child to follow all of the safety rules and the coaches' instructions.
Eligibility - I am my child agree to comply with the rules of Flipside Gymnastics.
Readiness to participate - I will allow my child to participate in those classes, events, competitions or activities for which I believe they are physically and psychologically prepared. I acknowledge my obligation to seek qualified medical advise if I am aware of any pre-existing conditions which my child may have that would limit normal participation and to make Flipside Gymnastics aware of any limitations placed on my child's participation.
Medical Attention - I hereby give my consent to Flipside Gymnastics, it's representatives and/or the host organization to provide, through medical staff of it's choice, customary medical/athletic training attention, transportation, or emergency medical services as warranted in the course of their participation. With the above in mind, and being fully aware of any risks, I agree to make my child aware of the safety issues and consent to have my child participate in programs offered at Flipside Gymnastics. I, my successors, or other representatives, waive and release all claims for damages that I or my child may have against Flipside Gymnastics and its representatives whether paid or volunteer, forever.
I, being the parent or legal guardian of the above child, have read and understood the above waiver and agree to abide by it.
